B. Requirements
1. Fully accomplished and notarized Application Form for Regular Membership
duly signed by the owner or its authorized representative.
2. Must be a CABLE TELEVISION (CATV) OPERATOR and holder of Provisional
Authority (PA) or Certificate of Authority (CA) to operate a Cable TV System in
the Philippines from the National Telecommunications Commission (NTC).

TV)
a. Submit photocopy of Provisional Authority (PA) or Certificate of Authority
(CA) to operate a Cable TV System in the Philippines from the National
Telecommunications Commission (NTC).
b. Must be operating its cable television system and have complied with all
the requirements such as:
- NTC CERTIFICATE OF REGISTRATION/TVRO
- LGU Business Permit
c. For Corporation, a photocopy of the Articles of Incorporation and By-laws,
together with the Certificates issued by the Securities and Exchange
Commission.
For Partnership, a photocopy of Articles of Partnership and business
registration certificate with the Department of Trade and Industry (DTI).
For Single Proprietorship, a photocopy of business registration and
certificate with the Department of Trade and Industry (DTI).
d. A certificate signed by the secretary of the Corporation/Partners/Owner
authorizing the filing of application for membership with the PCTA,
including the designation of an authorized representative of the
Corporation/Partnership/Proprietor to the Association.
3. Payment of applicable annual membership fee.
AFFILIATE REGISTRATION
An Affiliate Member of the PCTA is entitled to all notices, announcements and
communiqués from the Board of Directors of the Association and other information
regarding the cable television industry to be published by the Association.
A. LOCAL
1. Fully accomplished Application Form for Affiliate Membership duly signed by
the applicant or its duly authorized representative (in duplicate form).
2. For Corporation, photocopies of Articles of Incorporation and By-Laws together
with the Certificates issued by the Securities and Exchange Commission (SEC)
and Department of Trade and Industry (DTI).
For Partnership, photocopies of Articles of Partnership and Certificate of
Business Registration from the Department of Trade and Industry (DTI).
For Single Proprietorship, a photocopy of business registration and certificate
with the Department of Trade and Industry (DTI).
3. Certificate signed by Authorized Official of the Corporation/Partners/Owner
Authorizing the filing of application for membership with the PCTA including
the designation of an authorized representative of Corporation/Partnership/
Proprietor to the Association.
4. Payment of applicable annual membership fee.
B. FOREIGN
1. Fully accomplished Affiliate Membership application form duly signed by the
applicant or its duly authorized representative (in duplicate).
2. Payment of applicable annual Membership Fee.
